Job Overview

As a Virtual Registered Nurse, you'll play a vital role in our new patient care model, utilizing your clinical knowledge and critical thinking skills to coordinate and implement personalized care plans that meet the unique needs of our patients. This is an onsite position, working from a designated location within our hospital campus, and offers a unique opportunity to be part of a pioneering approach to healthcare delivery.

Key Responsibilities

• Utilize your clinical expertise to assess, plan, implement, coordinate, monitor, and evaluate patient care plans from admission to discharge.
• Leverage technology to provide additional support to our staff and enhance the patient experience.
• Apply the principles of care coordination and adult education to ensure seamless transitions of care.
• Collaborate with healthcare teams to develop and implement individualized care plans that address the complex needs of our patients.
• Work effectively in a fast-paced environment, prioritizing tasks and managing multiple patient cases simultaneously.
• Maintain accurate and comprehensive records of patient interactions and care plans.

Requirements and Qualifications

Essential Qualifications

• Current licensure as a Registered Nurse in the State of Idaho, as defined by the Idaho State Board of Nursing.
• Minimum eight (8) years of experience in health condition management, with a focus on adult condition management.
• Electronic Medical Record (EMR) experience.
• Graduate of an accredited school of nursing.

• Bachelor of Science in Nursing degree.
• Experience with care coordination and patient care models that leverage technology.
