As a Home Health Physical Therapist with Interim HealthCare, you will have the opportunity to work with patients who have lost mobility, strength, and functionality due to illness or injury. Key Responsibilities

  • Provide home-based physical therapy to patients, utilizing evidence-based practices and techniques to promote recovery and rehabilitation
  • Conduct comprehensive assessments to identify patient needs and develop individualized care plans that align with their goals and objectives
  • Collaborate with other healthcare professionals, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to ensure a cohesive and patient-centered approach to care
  • Observe patient movements, document progress, and modify care plans as needed to ensure optimal outcomes
  • Assist patients with exercises and hands-on therapy to manage pain, increase mobility, and enhance functional abilities
  • Educate patients and their families on the plan of care, established goals, exercises, and self-care strategies to promote independence and self-management
  • Coach family members on how to safely assist with exercises and support their loved ones in achieving their therapy goals

Essential Qualifications

To be considered for this exciting opportunity, you must possess the following essential qualifications:

  • Graduate of an accredited Physical Therapist Program and active PT license in IL
  • Minimum of 2 years of physical therapy experience, ideally in home healthcare
  • BLS and CPR certifications
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion

Preferred Qualifications

While not required, the following preferred qualifications will enhance your candidacy:

  • Experience working with patients with a variety of diagnoses, including orthopedic, neurological, and cardiovascular conditions
  • Strong communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams
  • Proficiency in electronic medical records (EMRs) and other healthcare technology
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care
