We're seeking an experienced and compassionate Case Manager to join our team in Fauquier and Southern Prince William County, with the flexibility to work remotely. As a Case Manager, you'll play a vital role in ensuring the well-being and satisfaction of our clients, while enjoying a competitive salary and comprehensive benefits package.

Key Responsibilities

  • Monitor services provided to individuals on the CCC+, CL & FIS waivers, as well as EPSDT program recipients, to ensure their safety, satisfaction, health, and welfare while living independently with support in the community.
  • Conduct monthly home visits (maximum) to assess client needs and provide support.
  • Utilize electronic devices and associated tools and equipment to complete day-to-day tasks, working remotely from a home workspace.
  • Maintain accurate records and reports, using a variety of applications and programs, including Office, Google, and Adobe.
  • Communicate effectively with clients, families, and healthcare professionals, both verbally and in writing.
  • Submit to a criminal background check and provide two professional references.

Essential Qualifications

To be successful in this role, you'll need:

  • A documented combination of skills and relevant work experience in a human services field.
  • Person-Centered Thinking/Planning training is highly desirable.
  • A valid driver's license and a personal vehicle safe for use as needed.
  • Good written and verbal communication skills.
  • One of the following: An associate's Degree or higher in a human services field, or both a Bachelor's degree or higher in a non-human services field and three years of relevant experience.

Preferred Qualifications

While not essential, the following qualifications will be considered an asset:

  • Experience working with individuals with disabilities.
  • Knowledge of Medicaid waivers and EPSDT programs.
  • Proficiency in using electronic devices and associated tools and equipment.
