Key Responsibilities
As a Patient Care Manager, your key responsibilities will include:
- Conducting comprehensive assessments to evaluate the medical, psychological, and social requirements of patients
- Collaborating with a multidisciplinary team, including physicians, pharmacists, and other healthcare professionals, to formulate and execute tailored care plans
- Delivering ongoing education on chronic disease management, encompassing medication adherence, vital sign monitoring, nutrition, exercise, and lifestyle changes
- Facilitating effective communication among patients, caregivers, and healthcare team members to ensure coordinated care
- Monitoring RPM data to detect trends and potential health risks, sharing insights with relevant healthcare professionals
- Maintaining accurate documentation of patient interactions and care activities in the electronic health record (EHR) system
- Assisting with the enrollment of eligible patients into the CCM and RPM programs while assessing program effectiveness and patient satisfaction
Required Skills
To be successful in this role, you will need to possess the following skills:
- Proficiency in electronic health records (EHR) systems
- Familiarity with Google Workspace
- Strong interpersonal and communication skills for effective patient education and teamwork
- Excellent organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously
Qualifications
We are seeking a candidate with the following qualifications:
- Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) is required
- At least 2 years of experience in patient care, care coordination, or chronic disease management
- Experience with CCM or RPM programs is advantageous but not mandatory

Compensation and Benefits
