Job Summary:

Key Responsibilities:

  • Utilize persuasive communication skills to overcome obstacles and close gaps in care, ensuring patients receive the necessary support and resources
  • Maintain accurate and up-to-date records of patient interactions, using CRM tools to track and analyze data
  • Develop and maintain strong relationships with patients, healthcare providers, and internal stakeholders, ensuring a high level of satisfaction and trust
  • Stay up-to-date with industry developments, regulations, and best practices, applying this knowledge to improve patient outcomes and service delivery
  • Participate in quality improvement initiatives, contributing to the development of new processes and procedures that enhance patient care and service excellence
  • Collaborate with cross-functional teams to identify and address patient needs, ensuring a seamless and coordinated care experience

Essential Qualifications:

* Bachelor's degree in a related field (e.g., healthcare, business, communications)

  • At least 2 years of experience in a healthcare setting, with direct communication with patients and healthcare providers
  • Proven track record of delivering exceptional customer service, with a strong focus on patient satisfaction and outcomes
  • Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced environment
  • Strong analytical and technical skills, with experience using CRM tools and other software applications
  • Ability to work independently, with a high level of autonomy and accountability
  • Strong attention to detail, with a focus on accuracy and precision in all interactions and communications

Preferred Qualifications:

* Bilingual or multilingual skills, with the ability to communicate effectively with patients and healthcare providers in multiple languages

  • Clinical experience or certification (e.g., CNA, LPN, RN) is highly desirable
  • Experience working in a call center or customer service environment, with a strong focus on patient engagement and satisfaction
  • Familiarity with HEDIS screening measures and conduct change initiatives is a plus
