Clinical Evaluator (PRN) - Pre-Admission Screening and Resident Review (Hybrid | Shawnee Kansas)

About the position Acentra Health is looking for passionate Clinical Evaluators (PASRR) to join our growing team! Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Shawnee, Kansas. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Shawnee and its surrounding areas. As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes. This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses-including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.

Responsibilities

  • Conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on clinical assessments.
  • Conduct assessments in various environments, including nursing homes, hospitals, and community settings.
  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations.
  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks.
  • Leverage technology by utilizing the state's electronic system and proprietary platforms to manage and track assessment activities.
  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Ensure compliance with necessary workflows to maintain quality and efficiency.
  • Efficiently prioritize and execute tasks with attention to detail.

Requirements

  • Active, unrestricted licensure in the state of Kansas in one of the following fields: LCSW, LMSW, LMFT, LICSW, LCP, RN, or another qualified LMHP.
  • Graduation from an accredited college or university with a degree relevant to the field, and minimum of 2+ years of experience assessing and diagnosing individuals with mental or psychiatric disorders.
  • Minimum 1 year of experience working with individuals with intellectual disabilities or related conditions.
  • Working knowledge of the Preadmission Screening and Resident Review (PASRR) requirements as per Federal Regulation and the state PASRR rules.
  • Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning.
  • Knowledge of psychiatric disorders as outlined by the latest Diagnostic and Statistical Manual in use in Kansas.
  • Strong communication skills with the ability to interact effectively with individuals and groups.
  • Detail-oriented, with the ability to prioritize and execute tasks efficiently within established timelines.
  • Proficient in using computer-based software and Microsoft Office Suite.
  • Familiarity with public mental health and medical programs is a plus.
