They are seeking a self-motivated and detail-oriented Bilingual Analyst, Case Management to assess, plan, implement, and coordinate case management activities with members to enhance their overall wellness.

Responsibilities

• Assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
• Managing a caseload that includes supportive and medically complex members
• Developing a proactive course of action to address issues presented to enhance short and long-term outcomes
• Determining appropriate services and supports due to member's health needs, including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Skills

• Bi-lingual (English and Spanish)
• Must reside in zip code 33614, or Surrounding Area within 2-3 miles
• Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL
• 1+ years of experience in behavioral health or a long-term care setting
• Computer proficiency in Microsoft Word, Excel, and Outlook
• Bachelor's Degree required, preferably in Social Service or related field (social services field - psychology, health administration)
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed care experience
• Case management and discharge planning experience
