- Proven experience as a Behavioral Health Specialist or Clinical Social Worker in a clinic setting

Responsibilities
- Conduct initial assessments to understand patients’ mental health status and identify their specific treatment needs
- Collaborate with primary care providers, psychiatrists, psychologists, and other team members to develop and implement personalized treatment plans
- Provide brief interventions, counseling, and psychoeducation to empower patients
- Monitor patients’ progress and treatment adherence through regular follow-ups
- Coordinate care transitions and referrals to specialized mental health services
- Educate patients and their families about mental health conditions and available resources
- Maintain detailed and accurate patient records in our electronic health record (EHR) system
- Participate in case load reviews, team meetings, and quality improvement initiatives
- Collaborate with external mental health providers and community organizations
- Adhere to the highest ethical and legal standards
Qualifications
- Bachelor’s Degree in Sociology or Psychology
- Proven experience in the behavioral health field
- Ability to work collaboratively in a team environment
