Experienced Licensed Social Worker – Remote Medical Case Management

Job Summary
We're seeking an Experienced Licensed Social Worker to join our remote team. As a licensed Masters Social Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. Your population will span the entire lifecycle, and all member contact will be done telephonically or via electronic exchange. You'll empower members to make positive lifestyle and medical choices and work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Empower members to make positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
- Collaborate with other departments to ensure seamless care coordination and communication
Requirements
* Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work

Work Environment
* Remote work from home role
- Must be able to work anywhere in the U.S.
- Working hours will be Monday-Friday, 8am-4:30pm Pacific Standard Time
- No weekends or holidays will be required
