About the Role

We are looking for an Advanced Practice Provider (APP) who will be responsible for delivering virtual and telephonic musculoskeletal evaluation and triage. This individual will assess patients, identify red flags, determine urgency and appropriate next steps, and initiate medical decision-making within scope. The APP will have authority to order imaging, therapy, and specialty referrals, prescribe short-term, non-opioid medications when appropriate, and complete required documentation in accordance with state regulations. The APP will collaborate closely with Physical Therapists and Care Coordinators to support coordinated, value-based care focused on appropriate utilization and patient outcomes. This role reports to the Regional Medical Director.

What You’ll Do

  • Virtual Triage & Clinical Evaluation
  • Conduct virtual and telephonic musculoskeletal triage and evaluation appointments.
  • Identify red flags and determine appropriate level of urgency, escalation, and follow-up.
  • Develop and document evidence-based clinical assessments and care recommendations.
  • Medical Decision-Making & Orders
  • Order imaging, therapy, or specialty referrals when clinically indicated.
  • Prescribe short-term, non-opioid medications when appropriate and within the scope of practice.
  • Sign plans of care and complete required authorizations or documentation in accordance with state regulations.
  • Patient Education & Communication
  • Provide patient education regarding condition, prognosis, and recommended next steps.
  • Communicate results, recommendations, and follow-up plans in a timely and patient-centered manner.
  • Collaboration & Continuity of Care
  • Collaborate with Physical Therapists and Care Coordinators regarding care navigation, progression, adherence, and escalation.
  • Support coordinated, team-based care across the episode.
  • Documentation & Compliance
  • Maintain accurate, timely, and compliant documentation in the electronic health record.
  • Adhere to established clinical protocols, escalation pathways, and quality standards within a value-based care environment.

Must-Haves

  • Active, unrestricted Advanced Practice Provider (APP) license in the state(s) where care will be delivered.
  • Master’s or Doctoral degree in a relevant advanced practice clinical discipline from an accredited program.
  • National board certification appropriate to APP role and scope
  • Minimum of 3 years of clinical experience in orthopedic or musculoskeletal care (e.g., orthopedics, sports medicine, MSK-focused clinics).
  • Strong clinical reasoning skills with the ability to determine appropriate care pathways and escalation.
  • Ability to deliver high-quality care in a collaborative, virtual care environment.

Preferred

  • Experience delivering care via telehealth or telephonic evaluation.
  • Spanish language proficiency.
