- Monitor patient progress and report changes to the supervising physical therapist.
- Educate patients and families on home exercise programs and safety techniques.
- Maintain accurate documentation of patient care and progress notes.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Ensure compliance with health care regulations and company policies.
- Provide compassionate support and encouragement to patients during therapy sessions.
- Travel to patients' homes within the service area as required.
- Active Physical Therapist Assistant (PTA) license in California.
- Experience in a home health care setting preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of physical therapy principles and practices.
- Current CPR and First Aid certification.
- Reliable transportation and valid driver's license.
- Compassionate demeanor with a patient-centered approach.
