Patient Tele-Outreach Representative
Job Duties
The Patient Tele-Outreach Representative contacts patients/parents/guardians to schedule appointments for immunizations and/or Well Child Checks (WCC) and documents call-back/mail efforts in the Electronic Health Record (EHR). The Patient Tele-Outreach Representative routinely uses databases, websites, and/or personal communication methods to request and retrieve pertinent records from outside organizations. Information/data gathered is compiled, entered, and verified for entry into the EHR. The Patient Tele-Outreach Representative works with the clinical team to meet or exceed national benchmarks for immunization and WCC rates for OneWorld patients. The Patient Tele-Outreach Representative is also involved in other activities related to vaccines and the promotion of health and wellness.

Contacts patients/parents/guardians to schedule appointments for immunizations and/or Well Child Checks. Documents call-back/mail efforts in the EHR.

Reviews pertinent records and determines what information needs to be entered into the EHR and/or other databases. Properly navigates through various screens and accurately enters the data.

Ensures that the data is correct. Verifies any missing or atypical information.

Initiates interventions to prevent individual patients from experiencing vaccine or WCC delays and/or brings those with delayed immunizations up-to-date.

Uses available resources to retrieve and enter historical information for individual patients in the EHR.

Performs other data-related duties as needed to meet or exceed monthly goals for immunization and/or WCC case management activities.

Tracks patient/parent/guardian tele-outreach activities as assigned.
