Key Responsibilities
- Conduct comprehensive assessments to identify patients' psychosocial needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide counseling and support for patients and their families dealing with grief and loss.
- Advocate for patients' rights and coordinate services to ensure holistic care.
- Maintain accurate documentation and case records in compliance with regulations.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited program.
- Active state licensure as a Clinical Social Worker (LCSW).
- Experience in hospice, palliative care, or a similar setting is preferred.
- Strong communication and interpersonal skills, with a genuine empathy for patients and families.
- Ability to work independently and manage multiple cases effectively.
