TFI Family Services is hiring a full-time Aftercare Case Manager to join our team! What Youll Need ? A bachelors degree in human services. ? A valid drivers license. ? A willingness to learnwell provide all the training you need! A Day in the Life As An Aftercare Case Manager, Youll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. Youll ? Focus on risk management and help families maintain stability during the aftercare period. ? Provide crisis intervention and work with public and private agencies offering supportive services. ? Maintain accurate documentation and reports. ? Coordinate community and educational services for families. ? Meet with families in-person to support reunification and resolve any concerns. What Were Looking For Were searching for someone who: ?
