Responsibilities • Assess, plan and implement individualized care strategies for patients. , • Identify and initiate referrals for social service programs, including financial and community support services. , • Manage the care plan as a single point of contact throughout the continuum of care. , • Communicate health-related information to all stakeholders to ensure quality coordinated care. , • Advocate for patients and families to ensure their needs and choices are represented and supported. Requirements • Active, unrestricted LVN/LPN license for the state of Texas. , • 2+ years of clinical experience in a health care role. , • Intermediate Microsoft Office experience, including Word, Excel, and Outlook. , • High-speed internet access in residence. , • Reliable transportation and ability to travel within the assigned region. Nice-to-haves • 1+ years of experience in a community health setting. , • Experience with electronic charting. , • Experience arranging community resources. , • Field-based work experience. , • Home health experience. , • Background in managing populations with complex medical or behavioral needs.
