Position: Hybrid Registered Nurse (Non-Clinical) Location: New York Job Posting: UAS Assessment Nurse (Bronx, NY) Position: UAS Assessment Nurse Location: Bronx, NY Schedule: Hybrid (3 days in field, 2 days remote) Hours: 8:30am - 5:30pm, Monday - Friday (40-hour work week) Overview: Join our team as a UAS Assessment Nurse and play a crucial role in assessing members' healthcare needs and plan eligibility across downstate New York. This position offers a unique blend of remote work flexibility and field visits, providing an opportunity to make a meaningful impact in the community. Responsibilities: • Conduct field assessment visits in the Bronx and surrounding areas, approximately 2-3 visits daily. • Complete documentation required by the Department of Health (DOH) to determine members' enrollment, service needs, and plan eligibility. • Coordinate with the Care Management Team (CMT) to ensure efficient and effective operations of long-term care plans. • Perform remote documentation and paperwork after field visits from home. • Utilize provided laptop and equipment for job-related tasks.
