As a Field HCBS Wait List Care Coordinator, you will be responsible for developing and implementing person-centered, goal-oriented care plans that address the unique needs of individuals receiving home and community-based services. Your expertise will ensure that our clients receive timely, effective, and cost-efficient care, while also promoting their independence and well-being.

  • Flexible scheduling, with a standard Monday-Friday schedule between 8am-5pm
  • Local travel required (up to 75%), with mileage reimbursement at the current government rate

Primary Responsibilities

  • Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Ensure care plans address social determinants of health, covered services, and value-added benefits
  • Communicate effectively with stakeholders to ensure quality, coordinated care and services
  • Advocate for individuals and families, ensuring their needs and choices are fully represented and supported
  • Conduct home visits, in-person visits, and serve as a resource for community care coordinators

Required Qualifications

  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services field
  • 1+ years of experience working with individuals with long-term care needs
  • 1+ years of experience working in a community health setting in a healthcare role
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications
  • Ability to travel 75% of the time, with a valid driver's license, current automobile insurance, and reliable transportation
  • Access to a designated quiet workspace in your home, with a secure internet connection

Preferred Qualifications

  • Licensed Social Worker or clinical degree
  • Experience working with individuals receiving services on one of the home and community-based waivers in Kansas
  • Background in managing populations with complex medical or behavioral needs
  • Experience with electronic charting and arranging community resources

  • Demonstrated ability to work well with others, with strong organizational and time management skills
  • Ability to multi-task, prioritize tasks, and meet deadlines in a fast-paced environment
