Job Description

We're looking for a dedicated and empathetic CareCoach to provide patient-centered care, support care plans, and coach individuals on maintaining a healthy lifestyle. As a CareCoach, you'll:

• Provide empathetic support and education to patients with chronic conditions
• Develop strong relationships with patients through regular communication and follow-ups
• Coach patients on healthy lifestyle choices, including diet, exercise, and medication adherence
• Encourage and guide patients in using vital monitoring tools to manage their health effectively
• Coordinate patient care across multiple providers and services to ensure comprehensive care
• Work efficiently with various Electronic Medical Records (EMR) systems, including EPIC, eClinicalWorks, and Practice Fusion

Requirements

• Certified Medical Assistant (CMA) certification required
• Experience in chronic care management and patient education
• Strong understanding of various chronic conditions and their management
• Proficiency in using EMR systems
• Excellent communication and interpersonal skills
• Ability to work independently in a remote setting
• Passion for patient care and curiosity about advancements in AI technology
• Bilingual proficiency in Arabic or Spanish strongly preferred
