Job Description: The RN Case Manager will provide personalized, telephonic care coordination for patients on specialty medications. You’ll be the empathetic voice patients turn to for medication education, follow-up support, and help navigating insurance or healthcare barriers. Bilingual fluency in Spanish and English is required to serve our diverse patient population effectively.
