Who You Are: Amoskeag Health is seeking a part-time Remote Triage Registered Nurse (RN) who is skilled in clinical assessment and able to provide timely, equitable, and high-quality care as the first clinical point of contact for patients via telephone or digital platforms. You are comfortable managing a high volume of diverse patient concerns with excellent judgment and professionalism. You understand the unique challenges faced by patients in a Federally Qualified Health Center (FQHC) setting, including barriers related to language, insurance, transportation, and chronic illness. You are committed to delivering accessible, trauma-informed, and culturally competent care, ensuring patients receive appropriate guidance, education, and support through virtual channels. What You'll Do: Clinical Assessment & Telephone Triage (Remote)
- Deliver timely and thorough telephone triage using evidence-based protocols.
- Identify and respond to urgent symptoms, escalating to providers or directing emergency services as needed.
- Determine appropriate care pathways (e.g., telehealth visit, urgent care referral, self-care advice).
- Assess for and document social determinants of health that may affect follow-up or care access.
- Provide trauma-informed, culturally competent care in all interactions.
- Reinforce provider instructions, educate on symptoms, and promote self-management.
- Document all triage interactions clearly and accurately in the Electronic Health Record (EHR)(Athena).
- Provide follow-up calls to high-risk patients as directed.
Remote Patient Care Coordination
- Monitor and respond promptly to digital communications (e.g., internal messaging systems like Team Urgent).
- Coordinate escalations and triage concerns raised by non-clinical staff in real-time.
Health Promotion & Education
- Integrate preventive education into every remote interaction.
- Deliver age-appropriate education on chronic disease, medication use, lifestyle modifications, and vaccinations.
- Promote health literacy through clear explanations of clinical concepts in understandable language.
Documentation & Compliance
- Ensure all clinical encounters are fully documented in the EHR (Athena).
- Meet all compliance standards set by the NH Board of Nursing, HRSA, and internal quality metrics.
- Track and report patient deferrals or barriers to care for quality improvement purposes.
- Strictly uphold HIPAA and all patient privacy laws in remote settings.
Remote Work Requirements
- Maintain an active NH RN license and comply with NH Board of Nursing telehealth regulations.
- Demonstrate sound triage judgment, protocol adherence, and documentation quality to sustain remote eligibility.
- Adhere to remote productivity standards and maintain professional communication.
- Participate in skills assessments and triage audits.
What You'll Bring:
- Graduate of accredited nursing program (RN), BSN required.
- Registered Nurse (RN) license required in NH.
- BLS certification required.
- 3–5 years clinical RN experience required, with at least 2 years in a telephone triage, urgent care, or acute care setting.
- Ability to prioritize and manage high call volume.
- Excellent verbal and written communication.
- Must be able to respond quickly to onsite clinical emergencies.
- Proficient in EHR systems and basic medical technologies.
- Bilingual (Spanish/English) highly preferred.
- FQHC or community health experience preferred.
- Experience with pediatrics, OB/GYN, geriatrics, and chronic disease management preferred.
- Ability to work onsite for the first month for training/competencies before transitioning to fully remote required.

- Compensation for this role is $36-$44/hour.
- This position is Part-Time 24 hours, 8am-5pm (M,W,F).
- This position is non-benefits eligible.
