Responsibilities
As a remote medical social worker and case manager, your responsibilities will include:
- Partnering with registered nurse case managers to assess, plan, and advocate for members' benefit plans and health needs
- Researching available options and resources for members with financial, transportation, safety, and housing needs
- Empowering members to make positive lifestyle and medical choices
- Working as a team with other clinical staff to connect members with community resources for medical, financial, and behavioral health
- Conducting telephonic or electronic exchanges with members to assess their needs and develop plans to address them
- Collaborating with internal stakeholders to ensure seamless care coordination and communication
Qualifications
To be successful in this role, you'll need:
- An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- A Master's degree in Social Work
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Proficiency in electronic medical records and other healthcare software systems
