Job Summary

We're seeking an experienced Licensed Social Worker to join our remote team as a Medical Social Worker and Case Manager. In this role, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs, as well as empowering members to make positive lifestyle and medical choices.

Responsibilities

As a Medical Social Worker and Case Manager at arenaflex, you'll have the opportunity to make a real difference in the lives of our members. Your key responsibilities will include:

• Researching available options and resources for members with financial, transportation, safety, and housing needs
• Partnering with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
• Empowering members to make positive lifestyle and medical choices
• Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conducting telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
• Maintaining accurate and up-to-date records of member interactions and progress

Qualifications

To be successful in this role, you'll need to have:

• An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
• At least 2 years of experience in medical social work or case management
• A Master's degree in Social Work or a related field

Compensation and Benefits

We offer a competitive salary range of $46,597.20 - $101,228.40, depending on experience and qualifications. In addition to your base salary, you'll also be eligible for a CVS Health bonus, commission, or short-term incentive program. Our comprehensive benefits package includes:
