GHR has a wonderful Care Management RN opportunity for a Registered Nurse working remote in the PA TRI STATE AREA! MUST have a PA, NJ or DE license Registered Nurse Job Details:
- Pay Rate: Starting at $40/hour
- Shifts Available: Full Time 9a-5p
- Infusion Therapy Team
- Perform telephonic or onsite review of hospital admissions
- Encourage efficient and high quality use of the healthcare delivery system
- Promote timely and dynamic discharge planning to facilitate early discharge
- Efficiently collaborate with the hospital utilization review department
Registered Nurse Job Requirements: (PLEASE only apply if you meet these requirements)
- Minimum 3 years of acute care clinical experience in a hospital or healthcare setting
- Prior discharge planning and/or care management experience REQUIRED
