We are seeking an experienced and skilled Physical Therapist II to join our Home Health team at Marian Regional Medical Center, part of CommonSpirit Health. As a Physical Therapist II, you will play a vital role in providing expert medical care, symptom and pain management, and emotional and spiritual support to patients in the comfort of their own homes.

Position Overview:

• Start Date: Immediate openings available
• Compensation: Competitive salary range: $44.73 - $64.85 per hour
• Location: Santa Maria, CA (remote work options available)

Job Summary:

As a Physical Therapist II in our Home Health team, you will work collaboratively with a multi-disciplinary team to develop and execute personalized care plans, identify functional goals and outcomes, and increase independence for our patients and their families. You will have the autonomy to work independently, participating in care coordination with the treatment team, physicians, and referral sources.

Responsibilities:

• Examine and evaluate patients to develop and execute a plan of care in collaboration with the physician
• Work with patients, their families, and the care team to achieve recovery and re-engage the patient with their community
• Participate in care coordination with the treatment team, physicians, and referral sources
• Manage audits, feedback on evaluations, and percepts

Qualifications:

• Minimum of 2 years of clinical experience within the last 5 years
• Graduate of an accredited physical therapy program
• Current California license in Physical Therapy (LIC PT)
• Current CPR through American Heart Association Certification (CT CPRBLS)
• Current California Driver's License (CA DL) with a clean driving record and proof of liability insurance
