Experienced Full-Time Remote Hospice Care Nurse – Compassionate Patient Care Specialist

Overview

As a Hospice Care Nurse with Amedisys, you will be part of a vibrant community that is dedicated to delivering high-quality, patient-centered care. Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
• Record pain/symptom management changes/outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with clinical manager and care team.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

To be successful in this role, you will need to possess the following qualifications:

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
